PLEASE NOTE - The Waukegan City Clerk's Office only maintains birth records for a period of ten years from the date of birth.
For birth records beyond this time period, please contact the the Lake County Clerk's Office via this link: http://www.co.lake.il.us/cntyclk/vital/birth.asp

Access to birth records is restricted. Birth records are available only to:

The person named in the record (if at least 18 years of age)
The parent of the person named in the record
A legal guardian who provides proof of court appointment
A legal representative who has notarized authorization from the person named in the record or the parent
*Exceptions to these restrictions apply to those interested in Genealogical Research.

Costs for the records are:

$6.00 for the first copy
$2.00 for each additional copy
